What website or strategy would you recommend someone to start using to start getting out of debt?

My doctor mentioned that it’ll cost $160 to fill out the form and the chances of getting approved is pretty slim despite me having ODSP. Is this true? Also, do you have to pay for your doctor to fill out the form? Online it mentions it’s suppose to be free but I’m not sure.
