I noticed my Focalin XR not working as well during and after my COVID infection, so I have been thinking about what could explain that change. Some people immediately point to manufacturer or generic differences, and that may explain part of what people are noticing. But another question is worth considering. What if COVID changed how our brain dopamine system works in some people, so that stimulants do not feel as strong or effective as they used to?

Several studies point in that direction. One found that SARS CoV 2 can affect dopamine producing brain cells and trigger inflammation and stress in areas that control focus and motivation (Yang et al., 2021). Another study showed that infected dopamine neurons make and release less dopamine and have disrupted systems for handling dopamine properly (Cappelletti et al., 2023). A broader Long COVID review also found dysfunction and inflammation in the basal ganglia, a key brain system that uses dopamine to control motivation, effort, and starting tasks. This is the same system stimulant medications are meant to boost (Kuczaj et al., 2024).

Taken together, these findings support a straightforward idea. Stimulants depend on a healthy dopamine system. If that system is inflamed, less efficient, or dysregulated after COVID, the same dose could feel weaker or less consistent. This does not mean Adderall has stopped working or that everyone is affected, but it does make it plausible that something may have changed in us, not just the medication, and that it is worth looking into further.
