Posting this as a reference for anyone who runs into something similar. I’ll update once I get an outcome.

I’m applying for my first adult UK passport from overseas. I was born in 1996 and my parent was born in the UK in 1967, so my claim is through my mother, not through my grandparents.

The case became complicated because my parent was adopted in the UK, later moved to Australia as a child, and used different names over time without a straightforward formal name-change document.

Because of the name mismatch, I had to go through a birth certificate amendment process in Australia before sending documents to HMPO.

What I had to do:

  • Request my parent’s UK birth record through the GRO
  • After multiple attempts, the GRO explained that because my parent was adopted, the original birth record was not available in the normal way
  • Request the UK adoption certificate instead
  • Apply to have my Australian birth certificate amended
  • Correct an error in my parent’s surname on my birth certificate
  • Add an official notation/endorsement linking my parent’s earlier UK identity to the later name used in Australia (This was ridiculously painful)
  • Get the corrected Australian birth certificate issued
  • Request my mothers official marriage certificate not the ceremony one.

Time frames / things to know:

  • UK adoption certificate: this can take time, so request it early or priority
  • Birth certificate notation/amendment: mine took up to about 7 weeks (And 20 emails of me fighting them for it, thanks NSW births deaths and marriages)
  • Marriage certificate: priority processing took about 2 weeks (Has to be requested by your parent)

Documents I’ve provided/sent:

  • Corrected Australian birth certificate with the notation/endorsement
  • Parent’s UK adoption certificate
  • Parent’s statutory declaration linking the different names
  • Colour copy of my current Australian passport
  • Additional supporting documents showing the name chain over time (Marriage cert shows her previous name)
  • A cover letter attempting to explain.

A few issues I ran into:

  • The passport application system initially asked for grandparent documents, even though my claim is through my parent and I don't have a father recorded on my birth cert so I cannot provide details.
  • I contacted HMPO/management staff for clarification but didn’t get a clear answer (After about 15 emails, all different staff members)
  • After I contacted them, the system oddly showed my documents as “received” before I had actually sent them

Has anyone else had a similar case involving adoption, parent name mismatch, no formal name-change document, or needing an amended birth certificate/notation to link the identity chain?

I’ll update once HMPO either accepts the documents or asks for more evidence.
