This is technically my second survey but I got a lot of comments of my last one that it was not done well, which I agree with. So, I have essentially remade it using their feedback. The main problem is that the survey style I am using now does not allow for a question to be given depending on how another question was answered, so they're all in the same form together. Sorry about that.
