Employer withholding benefit rate calculation

Kansas City, KS USA

My company recently switched to what my coworkers and I are referring to as “subsidized healthcare” where the rate you pay for a plan is dependent on your income. We all receive access to the same benefits, but the employee pays more out of pocket if they make more money. However, it makes no sense how they are coming up with the numbers and when I asked HR to explain the tiers or how rates are calculated, they gave me a canned answer from the benefits handbook that my rate was calculated from my salary + bonus, averaged over 2 years. I asked again for some sort of tier list or a way to calculate how my benefits would change if I was given a raise and was told by HR that they do not plan to release this information. Can they do that? My office is in Kansas if that matters, but this benefits plan applies to every US employee regardless of location.
