Have you ever gone through the effort of requesting recent appraisals after confirming positive reference checks (post-interview), only to NOT submit a package for a tentative offer?

Assume all references were positive and appraisals were between fully successful to outstanding. Have you ever requested all the above from someone, but then had reason not to submit them to HR for a tentative offer (i.e. your practice is to do this for multiple candidates just in case your first pick turns down offer or doesn't work out for whatever reason)
