Higher Level Examiner Rationale

I just received my decision letter for a claim of migraine, secondary to anxiety. What through the notes I noticed the examiner rationale paragraph and what was wrote in here was something that I did not say during my exam.

Has anyone ever had that issue and if so, how did you go about getting that corrected? Did you do a supplemental claim or did you try a board appeal with evidence submission?

Are there any Raters in here that might be able to chime in, please?
