Hello, I am in China and Entry-Exit require a document (同一人证明 Same person certificate）from the US consulate stating that my wife's old passport with maiden name and current passport with married name are the same person/identity. Great if anyone can share their experience with this, or what information to include in an affidavit form to give to a US consular official (the consulate will not give guidance on the wording of this document). Thanks.
