When sending a pa, do I leave intermed auth type as nonspecific or select intermed authorization? then type in fax number in the intermed auth ID

As an rph, when a patient schedule a vaccine online and it pops up on my computer to verify the online questionaire form and I do. Then it ask for me to put in my npi which I also do but then it gives me a system error after I enter my npi and click submit. Anyone know why?

I'm new and want to know the steps to create a wellness day in connexus and how to put in data for cholesterol, glucose in connexus. Would very appreciate the help
