Has this happened to anyone else?

Looking for some advice because I feel like I'm being walked into a trap here.
I’ve got a legal prescription for medical cannabis. My company just handed me a stack of 20 daily "Fitness for Work" self-cert forms to get through. The form makes you sign off saying you're totally unimpaired, sticking to your prescription, and that you've fully complied with all workplace medication disclosure and health and safety policies.
The problem is, my bosses have explicitly told me not to tell the client whose site I'm actually working on. They said they're keeping it quiet because the client will drug test or pull me off the job, and they just want me to sign these daily sheets so I can keep working.
It feels completely backwards. They’re making me sign a legal declaration every single day saying I’ve done all my disclosures and followed the rules, while actively forbidding me from actually telling the site client.
If the client ends up doing a random drug test or something happens, I’m the one who's gonna take the fall for lying or hiding it, even though I'm just doing what my company told me to do.
Has anyone dealt with this before? Am I gona get completely screwed over if I sign these? Already looking into calling Acas tomorrow, but wanted to see what people reckon.
