Trends and TRT after cancer

To be clear, never been on trt. I had my left testicle removed in 1/2022, then had my first test level checked through Marek 2 months after surgery in 3/2022. I was happy with the results! Not super high but not low either. E2 and shbg seemed reasonable as well. Had my Drs check again in 2024 and each year after. Always tested in the AM, fasted, no drinking the day before, etc.

I am 33yo, active (lifting and sports), relatively good diet, 6'1, 182 lbs

Test results:

3/2022: total 462 ng/dl, free 14.28 ng/dl

Marek test showed estradiol 35.6 pg/ml, vitamin d 81.6 ng/ml, shbg 25.1 nmol/l

2/2024: total 595, free 19.1

2/2025: total 469, free 14.9

2/2026: total 309, free 8.8 (worst by far)

This coincided with a small cancer recurrence in a lymph node that required open surgery to remove lymph nodes in March of this year. None of the research I've done suggests this cancer recurrence would cause low testosterone but who knows. I want to get another test to make sure but I definitely have higher anxiety and lower libido than a couple years ago. Mostly want to get this out to ask advice from other TC survivors or guys in their early 30s who started getting low T rapidly.
