Bilateral Dermoid Cyst- pain managed by PT?

I, 30 with hopes of healthy fertility, went in for an MRI for an old hip labrum injury and found multiple masses on both ovaries. Ultrasound concluded that I had bilateral dermoid cysts. Yes I’m in pain from a previous hip injury, but I could not have stressed more that I am also in extreme pain in the pelvic region, which has increased significantly in the last few months. The MRI was for the hip but I have been stressing over this recent pelvic pain as well.
Before gyn appointment, nurses and primary care doctor advised me to reduce activity since I am high risk of torsion. Baseline pain discomfort ranges 4-7/10 but have had a few serious, and scary 10/10 pelvic pain moments, always around ovulation time. During this I can’t walk, can’t stand, sweating, stabbing pain in my pelvic area but typically lasts less than an hour. I didn’t understand what was happening, assumed it was my aging IUD/hormone issue.
The doctor I saw today basically told me I COULD have surgery, but high risk of damaging or removing ovaries. He said I’ve most likely always had these large dermoid cysts and they won’t get any bigger so my best option for pain management would be pelvic floor PT and another MRI in 6 months to ensure no growth. He suggested my pain was probably due to tight pelvic floor and old hip injury. He also said because I don’t have “emergency level” pain, I should be okay with this plan…
I feel like this totally contradicted all information I’ve gathered before this appointment. I’ll get a second opinion, but, has anyone not had surgery for larger dermoids and managed pain with PT?

Side note: curious if anyone out there had egg retrieval before their surgery.
