I’m trying to get my medical certificate filled out but I’m so confused. Do I only need my doctor to just fill out this one form (the one attached here) or do I need them to fill it out AND have them write an official letterhead statement AND have that translated? I’m going through the SF consulate.

Plz be kind I looked at the website and prior years questions on the Reddit and still can’t figure it out 😔
