Ventriculomegaly causing mass effect on pituitary and weirdly low ACTH results?

I recently consulted my endocrinologist about some of the recent findings of my brain MRI (I've had a couple follow ups to monitor stability over the last 2 years) mentioning compression of pituitary gland and splaying of my optic chiasm.

I had an endocrinologist appointment finally after a long time of waiting and a battery of tests just to ensure stability and no crazy hormonal issues that cause my dizziness, pressure feeling behind my eyes (no pain pretty non specific tbh kinda *feels* the same as when you cross your eyes except i see fine) low heart rate and every day fatigue.

One finding I thought was interesting and that I wanted to share with you all here is that:

- My ACTH levels were 1.0 pmol/L and in 2024 they were 8.9 pmol/L (this was historical records). The lab identifies anything under 10.2 as "normal"

- Cortisol in 2024 was 529 nmol/L (140-535 range) and my last test was 238 nmol/L.

So i guess my concern is that with ACTH dropping down to 1.0 is this something notable? I included cortisol too because I figure if ACTH dropped then cortisol should've dropped significantly too?

Happy to share more if it helps paint a picture, but also understand if this isn't anything to think about :)
