Hey everyone! I don’t work in the medical field, but I got curious about how realistic the shift structure is, especially since i’ve heard Emergency Med is one of the few specialties with structured shift work, so I wanted answers from people actually working in the ED.
In the show, it kind of feels like there’s a pretty clean split between day shift and night shift teams, with only occasional movement (like Mateo going to nights/Ellis coming to days next season). But I’m wondering how it actually works in real life.
For those of you in emergency medicine:
1. Do you have a fixed schedule (like mostly days or mostly nights), or do you rotate between day and night shifts regularly?
2. Is there actually a “day team” vs “night team,” or is that more of a TV simplification?
3. If you rotate regularly, does the frequency of the rotation depend on your seniority/role? (i.e Intern vs R2 vs Senior Resident vs Nurse vs Attending)
4. I’ve heard of swing shifts (like mid-day to evening, for example \~11 AM–11 PM or 1 PM–1 AM); Are those a common/regular shift to be assigned?
5. How does scheduling usually work overall? Is it monthly rotations, weekly, random, etc.?
I’m especially curious about how exhausting it is to switch between shifts and whether programs try to keep things consistent for circadian rhythm reasons.
Would love to hear what it’s actually like vs what we see on TV!
