multiroom setup for both streaming and a record player?

hello,

looking for some advice; i have an audio-technica AT-LP120XUSB, and would like to create a multiroom system (2 small rooms) that lets me play music from both that and a hardwire connected ipad (done with all the connectivity issues with speakers running spotify direct!). budget is about 2-3K for the whole system: 1 amp and 2 stereo speaker pairs for 2 rooms.

i was looking at sonos, but it only has 1 input; and a lot of the amps i've seen only have 1 set of outputs. it seems like as soon as i try to get 2 inputs and 2 independent outputs, the price points and power levels seem to jump dramatically. any ideas? thanks!

acute lower back episode; help me understand my MRI?

hello,

hoping for some help tonight; i have a history of lower back episodes, but today is the worst episode i've had by far. i just woke from a nap and can barely move, walking to the bathroom was a real struggle. my lower back is just incredibly tight, and every little movement seems to aggravate it. (it is not radiating down my legs though, also no tingling or bladder issues).

this episode started yesterday morning, after 4 days of extended driving and uncomfortable sitting on location for my job. i'm currently taking meloxicam and cyclobenzaprine, and had a shot of methylprednisolone earlier today; all that has helped in the past but today not so much. i also usually do regular back stretches and exercises, all of which help a ton, but have been lapse on those in the last month because of crazy work times. and right now i can barely move, much less get on the ground and try to stretch...

i'm extra frustrated because i had an MRI done 3 months ago, but have still not had an meeting with an orthopedic surgeon, as every time i schedule one they call me a few days before the appt to tell me they don't take my insurance so they are canceling. the insurance game is a whole other post...but in the meantime, can anyone here help me understand what the below even means, and what i should be doing tonight to get better?

i am a 50 yr old male, and i've had these episodes off and on for the last 3 years; have had chronic low grade pain for nearly 10 years.

EXAM:
MRI of the lumbar spine without intravenous contrast

CLINICAL HISTORY:
Low back pain.

COMPARISON:
None.

TECHNIQUE:

Multiplanar and multisequence MR imaging of the lumbar spine was performed without the administration of intravenous gadolinium. Imaging sequences include sagittal T1, axial and sagittal T2, and sagittal inversion recovery images.

FINDINGS:
There is no thickening or clumping of the visualized nerve roots. Vertebral body heights are maintained. There are mild degenerative endplate changes at L4-L5. There is a small Schmorl's node of the inferior endplate of L4. There are small anterior osteophytes at T12-1, L1-_2, and L4-L. There is disc desiccation at L4-L5 and L5-S1. There is loss of disc height at T12-L1 and L4-L5. There is grade 1 anterolisthesis at L4-L5, likely degenerative. There is curvature of the thoracolumbar spine to the left. The prevertebral and paravertebral soft tissues are unremarkable.

There is a small cyst in the right kidney. Axial images angled through the disc spaces demonstrate the following:

T12-L1: There is a small left paracentral disc protrusion and there are mild changes of the facet joints. There is no central canal stenosis or neural foraminal narrowing. The disc contacts the left L1 nerve root in the lateral recess.

L1-L2: There is no central canal stenosis or neural foraminal narrowing. There are mild degenerative changes of the facet joints.

L2-L3: There is no central canal stenosis or neural foraminal narrowing. There are mild degenerative changes of the facet joints.

L3-L4: There is an annular tear. There is a diffuse disc bulge with a small central disc protrusion and there are degenerative changes of the facet joints. There is no central canal stenosis or neural foraminal narrowing.

L4-L5: There is diffuse disc bulge with a left paracentral disc extrusion. There is inferior migration of the disc without sequestered disc fragment. There are degenerative changes of the facet joints with hypertrophy of the ligamentum flavum. These findings cause mild central canal stenosis and left lateral recess narrowing with impingement of the left L5 nerve root and mild right and severe left neural foraminal narrowing with impingement of the left L4 nerve root.

L5-S1: There is an annular tear. There is diffuse disc bulge with a small right paracentral disc protrusion and there are degenerative changes of the facet joints. There is no central canal stenosis. The disc contacts the right S1 nerve roots in the lateral recess. There is mild right neural foraminal narrowing.

IMPRESSION:

  1. Multilevel degenerative disc and facet joint changes.
  2. Disc bulges and disc herniations at multiple levels causing central canal stenosis and neural foraminal narrowing as detailed above.
