

Chronic back, testicular, pelvic, hip, abdominal pain with no clear diagnosis — looking for advice
Since December 2024, I have been dealing with chronic discomfort in my left testicle. At first, it felt almost like a sore muscle.
I went to my primary care doctor and was checked for a hernia, but no hernia was found. I was then referred for a testicular ultrasound, which also came back with no significant findings.
I later saw a urologist who performed a prostate exam and diagnosed me with prostatitis. I took all of the medication he prescribed, but it had no effect on the pain. I eventually saw another urologist, who told me that prostatitis was likely a misdiagnosis and that I instead had pelvic floor weakness.
I went through several months of pelvic floor therapy and consistently did the exercises at home, but unfortunately, they didn’t help with the pain either.
As I started commuting more frequently for work, I noticed that my left hip began to feel tight and imbalanced. It also started to feel like I was sitting on a golf ball. Both of these symptoms became significantly worse specifically when driving or sitting for long periods.
I dealt with it for a while, but in August 2025, while playing sports, I herniated a disc and developed sciatica on my right side. An MRI showed an L5-S1 disc herniation on the right side. I received an epidural steroid injection (ESI), which helped for about a month, but the pain eventually returned.
Around this same time, I also started experiencing pain in my lower right abdomen that becomes especially inflamed or painful after masturbation or sexual activity. This occurs alongside the ongoing pain in my left testicle.
I also began experiencing heartburn and acid reflux during this period. It is especially bad when my stomach is empty.
Over the past few months, I have had two abdominal/pelvic CT scans, and neither showed anything that explained my symptoms.
At this point, I have been dealing with multiple types of chronic pain for a long time and have seen numerous doctors and specialists without getting a clear answer. I have been taking ibuprofen for about a year and a half, and it is the only thing that seems to reduce the pain, even if only slightly.
I honestly feel like I am running out of options. I am exhausted from being in pain and from constantly trying to figure out what is wrong with me. I am also feeling increasingly hopeless and have been slowly giving up on life.
If anyone has experienced anything similar—especially testicular pain combined with pelvic/hip pain, pain that worsens with driving or sitting, abdominal pain after sexual activity/masturbation, or an L5-S1 herniation—I would really appreciate hearing about your experience. I’m looking for any ideas about what I should investigate next or what kind of specialist might be able to connect these symptoms.
