For those of you that have done psychological waivers: is there a template or memo form that you put your “memo from soldier explaining situation, extenuating circumstances, or anything else they would like relayed to the wavering authority”?

I’ve been trying to find out if there’s a specific memo or form that it goes on but haven’t gotten good answer. Thanks.
