Is this a authentic certificates

So I bought sport goggles at an optical and they said that crizal is the cheapest lens they have so we went with that. We asked if it has an authenticity card and they said yes, but we did not expect it to be like that.. since i also had an essilor glasses b4 and the authenticity card did not look like that especially since the lens was pricey so I just wanted to make sure.

They said it ws sapphire smth and its polycarbonated
