Panda cory, loses control when darting to surface

Timeline, I’ve had the pandas since April of this year.
Last Monday I fed bloodworms (i feed them once a week to every 2 weeks as a snack). Then I noticed my filter glitched and wasn’t running. It could have glitched sometime between Sunday evening and Monday evening. I cleaned it out, plugged it back in and it dumped a load of gunk back into the tank. Did a water change and cleaned everything up. Tested water and everything was normal. PH 8, Ammonia 0, Nitrites 0, Nitrates 5.

Same day I noticed a cory was barrel rolling in the water column and on the substrate would kind of wobble sideways back and forth while resting but would self correct. It can rest normally on the sand. However when it enters the higher waters or darts to the surface it goes a bit haywire. Video shows it after it sank back down after darting to the surface. It stayed like that for four minutes until I netted it. Once netted it stayed upright.
Fasted for 72 hours, no improvement. Symptoms have not worsened. It can rest normally and move normally. But when it darts to the surface it loses control. So moved it to a floating container at the top of the column yesterday, that keeps it from crashing out when going to the surface.

Tried an epsom salt bath yesterday but netting and moving to a separate container caused it to lose control and go sideways.
Moved it back to the floating container. This morning I tried another epsom salt in it’s container and that worked much better, no freak outs. Did a complete water change after and added kanaplex today.

Does this seem like a swim bladder issue? Infection? Parasites? Am I on the right track with kanaplex? Or should I be trying something else?
This is the only fish showing any type of symptoms, everybody else is fine.
