Struggling in CC with 2.1 GPA

Hey everyone, as the title suggests, I've been having some trouble with CC due to mental health and overall class difficulty. I had a 4.0 in high school but I chose to take the CC route for financial reasons. Now, I don't really know what to do. I'm aiming for HYPSM, but I also have targets in UCLA, Berkeley, and Caltech. I want safeties outside of T20s as well such as UMich, UVA, VLU, and Georgia Tech. I know these seem out of reach, but they were my targets in high school and I got accepted into a few of them before making the CC route decision.

I understand that the first thing I need to do is consider why I got such a low GPA, and I feel personally that the mental health issues have come from isolation in CC, meaning I think I would do better once I get into a T20 or similar. Any advice for what to do? I am about to finish my transfer apps, and I don't have any options for remedial. Thank you so much for any help!

EDIT: To clarify some questions, I am currently at DVC, a CCC, and trying to major in CS!

EDIT 2: I have been scouted recently by T50s (notably Purdue and Brown) and some lower tier universities but not full ride for lacrosse granted i maintain above a 2.0 as I finish out the semester. I’m obviously super grateful, but I’m really hoping to make it on the radar for my targets and reaches due to the ones I listed being better for CS!
