Severe OATS (3M/ml, 0% progressive motility, 0% morphology)

32M from Hyderabad, India. Looking for advice from anyone with severe male factor infertility, especially around IVF/ICSI outcomes and doctor recommendations in Hyderabad.

Latest semen analysis (3 days abstinence):

  • Volume: 2.5 ml
  • Sperm concentration: 3 million/ml
  • Total sperm count: 7.5 million
  • Progressive motility: 0%
  • Non-progressive motility: 10%
  • Immotile: 90%
  • Morphology: 0%
  • Impression: Severe OATS

Other findings:

  • Hormones largely normal:
    • FSH: 4.05
    • LH: 4.62
    • Prolactin: 10.1
    • Total Testosterone: 338
    • Free Testosterone: normal
    • SHBG: low
  • Ultrasound:
    • Reduced testicular volume (~7–8.5 cc)
    • Grade 1 bilateral varicocele
    • Normal blood flow/structure

Current medications:

  • Spercus (L-Carnitine + CoQ10 + Lycopene + Zinc) – twice daily
  • Maxoza-L sachet (L-Carnitine + Acetyl L-Carnitine + CoQ10) – once daily
  • Addyzoa – once daily

Questions:

  1. Anyone with similar numbers (especially 0% progressive motility and 0% morphology) who had success with IVF/ICSI?
  2. Did supplements/lifestyle changes significantly improve your reports after 2–3 months?
  3. Any good male infertility/andrology or IVF doctors in Hyderabad that helped in similar cases?

Trying to understand realistic expectations and avoid wasting time.

👉 One-liner: Low sperm count (~2M/ml) with normal hormones but reduced testicular size.

🧾 New Ultrasound Findings

Testicular volume: ~7–8.5 cc (both sides, reduced)

Grade 1 varicocele (both sides)

Tiny epididymal cysts (doctor said not significant)

Blood flow & structure: normal

🩺 Doctor Opinions (conflicting)

1️**⃣ In-person **doctor

Says it’s likely testicular-level issue

Prescribed:

L-Carnitine + CoQ10 based meds

Ayurvedic supplement

Plan:

👉 Take meds for 1 month

👉 Then review

2️⃣ Online consultation

Says:

“Semen report seems unlikely given normal hormones”

Suggested:

👉 Repeat semen analysis (possible lab/sample issue)

🤔 My confusion

Ultrasound shows reduced testicular size (which seems to explain low count?)

Hormones are normal

One doctor says treat + wait

Other says report may be wrong

Questions

Has anyone had normal hormones + low count due to smaller testis size?

Is 1 month of meds enough to see any change?

Should I repeat semen test immediately or wait 2–3 months?

Hi all, looking for some honest input from people who’ve been through this.

I’m 32M, generally healthy:

  • Don’t smoke or drink
  • Moderately active (but can be more consistent)
  • No major medical issues

Just got my first semen analysis and the results came back concerning:

  • Volume: 2.5 ml (normal)
  • pH, liquefaction: normal
  • Count: ~2 million/ml
  • Morphology: <1%
  • Motility not clearly listed but overall impression: 👉 Severe Oligo-Astheno-Teratozoospermia (OATS)

No prior reports to compare, so this is my baseline.

Some context:

  • Currently taking basic supplements (B12, multivitamin, fish oil), planning to switch to a more targeted fertility stack
  • No smoking/alcohol, but lifestyle could definitely improve (exercise, sleep)

I’ll be getting hormone tests tomorrow (FSH, LH, Testosterone, Prolactin, TSH) and will likely do a repeat semen analysis in a few weeks.

Questions

  1. Has anyone with similar numbers conceived naturally? What kind of timeline are we talking about realistically?
  2. Did you see improvement with lifestyle + supplements alone? If yes, how much improvement and in how long?
  3. At what point did you decide to move to IVF/ICSI? Trying to balance not panicking vs not wasting time.
  4. Anything specific I should check with the doctor? (I’m planning to ask about varicocele, hormone issues, etc.)

Update: Got my hormone results back

Hormone panel results:

  • FSH: 4.05 (normal)
  • LH: 4.62 (normal)
  • Prolactin: 10.1 (normal)
  • Total Testosterone: 338 ng/dL (within range)
  • TSH: 3.34 (within range)
  • Ferritin: 95.9 (normal)
  • SHBG & Free Testosterone: pending

From what I understand so far, hormones seem largely normal, so this might not be a hormonal issue.

Would appreciate thoughts from anyone with similar reports, especially if hormones were normal but sperm parameters were low.
