Am I responsible for this?

I’m in my first week supernumerary in A&E. I was in reception with another nurse. She said on handover to the night staff that all the obs are up to date. I clarified it with her and she said yes they’re all up to date. I’ve now found out that most of them were around 6 hours overdue and the senior sister has been informed.

This is far from my standards and never been in a position where my practice could come into question. I feel like I’ve let my patients down and now my team. I will learn from this and check this myself. I’m not really understanding why the two HCAs didn’t do them. I was busy doing all the meds, IVs and BMs etc

Can anyone give any reassurance or some thoughts please!

Should I be upset?

I’ve left the ward after three years. One of the nurses said ‘I’m mad that they haven’t got anything for you leaving when they have others who have been here for a shorter time.’

I don’t care but it made me think a little bit about it. Should I be upset?

Two nurses having 9 patients each and one nurse having 0?

For our night shifts, our band 7 has now changed how the ward is allocated. She has not involved us in this change and has implemented it without much discussion.

We normally have three band 5s on a night shift. Most of the patients are acuity 5 with airways, enteral feeds, multiple IVs etc

She now wants two of the nurses to have 9 patients each and one to ‘navigate.’ There is nothing for the third nurse to do. Of course they will help but I don’t see it’s fair for me and another nurse to have 9 patients. My workload will be considerably more than the nurse with 0. I can’t see how this is justified and surely keeping patient to nurse ratio low means safer care?

Anyone have any thoughts on this? A lot of us are annoyed. Someone has spoken up about it and has been shut down. One nurse claims that it’s because ‘the night staff aren’t doing their jobs properly.’ Which is a completely unfair statement. We all work really hard. Feel very under appreciated.

Any advice on working in A&E?

Been a surgical nurse for three years. I’ve done a few bank shits in A&E but looking for ideas/tips on managing my workload and maintaining patient safety. I’m confident in my clinical skills/judgement but being a perfectionist I’m worried I’ll miss something 👍
