Black bars on sides with a 4:3 crt

I'm not sure how to fix this. I've tried googling but all I see are people having this problem on HDTVs and not actual 4:3 crts. The settings pages are for Sega Genesis Collection which is the game I'm having issue with.

Other games work fine like Sonic Heros.
