Do your dreams shift focus to different parts of the world over time?

I only recently noticed that over time my dreams shift focus from specific locations to others over time. I may have 2 or 3 dreams focused on one central location, then several dreams focused around a nearby location, and continuing from there. It is like I'm exploring the world naturally and seeing new places. I do return to past locations, but usually the past location has changed in some way (usually in a positive light, such as being less run down and more people). It brings me excitement whenever I move to new locations. Last night I explored a location from a semi-recent previous dream in much more depth. The area was beautiful so I'm looking forward to exploring it and surrounding areas in the next dream!

For example: 2 or 3 dreams about exploring a hotel and mall -> 2 or 3 dreams exploring the mall and nearby downtown -> 2 or 3 dreams about exploring the nearby downtown and restaurant center -> etc.

Anyone else experience that too or know what I'm talking about?

Is it common for doctors to leave the appointment without communicating the appointment is over?

I notice that during an appointment the doctor will leave the room shortly after going through the prescription or run down without communicating we are done with the appointment. For instance, no verbal 'have a good day', 'goodbye', 'see you at our next appointment', 'head to the next room for checkout', etc. I don't even mean being friendly, more like not saying anything at all. I'll usually stay in the room for a few minutes more wondering if we're done, and then quietly poke my head out looking for a nurse or someone to indicate we're done. I've experienced this with several different doctors. Is that common, or maybe bad luck on my part? I get that they are busy and have the next appointment/task on their mind, but it confuses me whenever it happens.
