21 M, Living in Syria. Condom broke for a few seconds during insertive anal sex. Tested negative on Day 8. Need realistic risk assessment considering limited local healthcare access.

I am writing to get a professional risk assessment for two separate incidents. I am currently dealing with severe health anxiety, and I need to know how to handle this realistically given my local circumstances.
Important Context - I live in Syria:
- Access to advanced testing (like HIV RNA/NAT) is very limited and expensive.
- PEP is extremely difficult to obtain in our healthcare system.
- The most accessible HIV tests available to me are private lab rapid tests or 4th Generation Ag/Ab tests from venous blood.
- I cannot easily get PEP for my sister, and frankly, even getting her tested is complicated and socially sensitive here.
1. The Primary Incident (Sexual Exposure)
- Date of exposure: July 31st (Currently Day 16).
- My Role: Insertive partner (Top) in anal sex with a male partner.
- Partner's HIV status: Unknown.
- Circumcision status: I am circumcised.
- The Event: We were using a condom, but it broke right at the very beginning of insertion. I noticed immediately and pulled out within a few seconds. There was no continuous friction, and no ejaculation inside the partner.
- Post-exposure: I ejaculated outside a few minutes later, and I urinated shortly after getting dressed (which I read might act as a mechanical flush for the urethra).
- Testing: I took an HIV test at a private lab on Day 8 (August 8th). I don't know the exact generation of the test, but the result was Negative.
- PEP: I missed the 72-hour window, and given my location, it would not have been available anyway.
2. The Secondary Incident (Household Exposure)
- I had a bleeding gum.
- ِAfter that, I drank from a shared drinking straw.
- Shortly after, my sister drank from the exact same straw.
- She does not have any visible mouth sores, but my anxiety is making me worry about hypothetical micro-abrasions in her mouth.
- Realistic concern: Getting PEP or testing for my sister here is not feasible. I need to know if there's any actual medical justification to even try, given our limited resources.
My Questions for the Doctors:
- Risk Assessment: Given that I was the insertive partner, circumcised, the exposure lasted only a few seconds, and there was no internal ejaculation, how do you assess my actual HIV risk?
- Urethral transmission: Is it possible for HIV to enter through the urethral meatus in such a brief timeframe (literally seconds)?
- Testing timeline (realistic options): Given that my Day 8 test was negative but likely not conclusive, and that my realistic testing options are 4th Gen Ag/Ab from venous blood, when should I test for a conclusive result? Is Day 45 sufficient?
- Sister's Risk (most important for my anxiety): Is there any documented risk or medical justification for my sister to take PEP or get tested due to the shared straw incident, even with the hypothetical presence of my blood and micro-cuts in her mouth? Are there any documented cases of HIV transmission via shared straws/cups?
- Realistic advice: Given the healthcare limitations in Syria, what do you recommend I actually do moving forward?
