I was in the die machine power room I used my aether shroud and I got stuck somewhere and immediately died, I have self revive, tomb, and quick revive, I didn't down just died, any ideas what could have happened???

Do I need to max out every gun or just one of each class to unlock it? I'm trying to grind it for my mac 10 and sa12
