Consider an alternative timeline where Muta never cooperated with Kenjaku and just made his shot with Miwa.

There is no denying that both of them had feelings for each other, and Miwa definitely wouldn’t have judged Muta for his appearance and condition.

However, without making the Binding Vow with Kenjaku and Mahito, Muta never would have been healed (unless Yuta was somehow able to copy Idle Transfiguration and use it on Muta).

The question then arises: would their relationship have worked out in the long-term? How much would Miwa be able to interact with Muta? Would Miwa have to stay in the basement with him all the time? Are they even able to have kids together? How much longer would Muta survive in his condition?

When I really think about it, the logistics of their potential relationship without Muta being healed is hard to determine.
