[70M] Debilitating severe daily imbalance, severe random bloating, & stalled bowel movements ruining quality of life (15+ yrs). Vitals: 153/81 standing.

- Primary Complaint: Severe debilitating daily imbalance, extreme leg heaviness/tightness, random severe abdominal bloating, and stalled bowel movements are completely destroying his quality of life.
- Duration: 15+ years, progressively worsening (bloating started around age 45).
- Existing Medical Issues: None officially diagnosed; history of 30 years of intense chronic stress (now retired and peaceful for 2+ years).
- Current Medications & Supplements: Magnesium Glycinate (nightly), Probiotics, Pepsin GI, Iberogast (20 drops), L-glutamine. Currently beginning a trial of: Claritin 10mg (AM), Pepcid AC 20mg (PM), Vitamin C, Quercetin, DAO enzyme, and Alpha Lipoic Acid.
- Drink/Smoke/Recreational Drugs: No drinking, no smoking, no recreational drugs.

Patient Background: Posting on behalf of my dad. He has been suffering from a highly symptomatic, multi-systemic condition for over 15 years. Standard gastrointestinal, structural, and respiratory evaluations have all come back completely clean (negative endoscopies, colonoscopies, MRIs, sleep apnea studies, etc.). We are desperate for a direction that looks past his normal baseline test results.
The Life-Altering Symptoms: These issues are what make his day-to-day life unlivable:
- Debilitating Imbalance & Leg Heaviness: He cannot simply walk. He has to intensely mentally prepare himself just to stay upright and keep his balance, akin to an acrobat walking on a tightrope. Furthermore, his legs are extremely tight, and when walking, he describes the feeling as having a "100-pound weight" attached to his legs.
- Severe, Unpredictable Bloating: He experiences severe abdominal bloating on a weekly/daily basis with absolutely no rhyme or reason. Strict adherence to a low-FODMAP diet and avoiding standard triggers has only offered minimal, incomplete relief.
- Minimal, Incomplete Bowel Movements: He has very little bowel movement every day. What does come out is soft and the size of a finger. He is left with a constant feeling of incomplete emptying. Crucial note: He recently had one rare, complete bowel movement and immediately stated, "I felt like I was 20 years old again," indicating just how deeply tied his systemic fatigue is to his stalled GI tract.
Secondary/Accompanying Symptoms:
- Vitals: Supine Hypertension with Orthostatic Hypertension (Lying down: 142/75; Standing: 153/81).
- Neurological: Extreme morning exhaustion (wakes up feeling as though he worked all night), random heart racing, constant pressure in the brain, and an acute feeling of coldness when everyone else is warm.
- Sensory: A strict, painful burning sensation isolated to the soles of the feet (likened to frostbite).
- Notable Acute Incident: One isolated incident of fainting and breaking out in a full-body rash immediately after smelling a sharp chemical that no one else around him could smell. He has ongoing sensitivities to fragrances and temperature changes.
Our Current Working Hypotheses:
- Dysautonomia / Autonomic Nervous System Dysregulation: Potentially explaining the debilitating imbalance, heavy legs, orthostatic blood pressure spikes, and exhaustion. Strongly suspected to be triggered by his 30 years of chronic stress.
- Mast Cell Activation Syndrome (MCAS): Given the chemical fainting/rash incident, environmental sensitivities, and erratic gut issues.
- Pelvic Floor Dysfunction: Accounting for the mechanical bowel symptoms and the profound systemic relief he felt when he finally fully emptied.
Questions for the Physicians:
- Given how severe the imbalance, heavy legs, and stalled bowel movements are, does this clinical picture align with Dysautonomia and/or MCAS?
- Are there specific tests (e.g., serum tryptase, 24-hour urine for histamine, plasma fractionated catecholamines) or specialists that we should urgently pursue?
- Could the 153/81 standing blood pressure be hyperadrenergic POTS driving the profound imbalance?
