7 years post-op and I have never been happy with my meta. Need advice.

I had meta with UL and scrotoplasty without implants with Dr. Hadj-Moussa at U of M almost 7 years ago now.

My biggest concern with my meta is it sits very much between my legs. I had a fistula near the tip after the surgery that she addressed by just cutting the tissue between it and the urethral opening. As a result, my urine goes straight downwards at the end of my stream. Due to the location so far between my legs, I cannot stand to pee unless I want to urinate all over my pants and boxers. To me, this completely defeats the entire reason I had the meta in the first place. I also hate how it looks squished between my legs all the time and it's super uncomfortable because I sit on it all frequently. When I lie on my side, everything is 100% smashed between my legs and that's unfortunately how I sleep.

I'm pretty thin and she told me that there's really not much more that can be done. This has been having an increasingly negative effect on my mental health for years and I can't live with it anymore. What do you guys recommend?
