Given the sword changes PC's permanently, did you warn/ask your players out of game or just let it be a surprise?

Hey folks,

We're coming up to the end of the campaign. My party is about to enter the bleeding citadel, and I'm just trying to think ahead.

Has anyone continued from this campaign into Chains of Asmodeus? And if so how was it? It seems like a natural continuation given that we've got the world set out essentially, some of the party have a contract or contracts and it would be cool to experience. I'd still like to hear some opinions on CoA though.
