Grade 5 AE reported, but cause of death entered as unknown
I feel like I find this discrepancy during data cleaning far more often than I would expect. On the EOS/Death CRF, the site states that the cause of death is "unknown", but then you look at the AE log and there's a G5 SAE staring right at you.
I guess I'm genuinely curious how this happens? What does your data entry workflow look like that something like that just slips?
