Unexplained debilitating gastrointestinal issues

Hello doctors of Reddit. I am posting this on behalf of a friend as no specialists have been able to help him.
Medical history:
2023 started allergy shots for dust mites (seems to be the catalyst)
After the shot he was unable to breathe, was told he had asthma. He did not have asthma prior to this.
Recurrent ear infections put on 50mg steroids 3x/day for three weeks
Subsequently diagnosed with sleep apnea.
Lung specialist said lungs looked fine
After the first round of steroids he developed pain I'm the lower right quadrant of his abdomen.
After onset of abdominal pain, daily migraines followed.
Everything he eats causes abdominal pain and bloating
He basically lives on the toilet and has had to quit his job due to inability to work.
He goes through vicious cycles of diarrhea and constipation
MRI, ultrasound, CT scan, endoscopy and colonoscopy all returned "normal" results.
Dr says it's "just" IBS
He's barely eating anything due to fear of pain and hasn't lost any weight.
Had an MRI of his pituitary gland that was normal as well.
Is this actually "just" IBS? Are there any other tests or possible diagnosis he could be looking into?
We're located in Canada so healthcare is free but takes forever to get the ball rolling.
Any help is appreciated! He's been suffering for 3 years now.
