How to put PE on resume?

Just got my license, how should it be shown on resume?
Should “P.E.” be put after my name?
Should the state be included?
Should the license number be included?

Density of Cement?

The solution for the problem has the Solid Density of cement as 195 pcf. Where does that value come from? I can’t find it in any reference.

Problem is taken from NCEES Civil: Construction Practice Exam.
