I’ve only worked in FQHC and I have a friend who is looking to leave her DSO and come work at an FQHC. I explained that there is no production or collection you have to worry about as the provider. Our productivity is measured off of encounters and the department generates revenue per encounter whatever our negotiated encounter rate is with the state Medicaid. That seems simple to me and it’s likely due to working in this environment for so long. However she can’t wrap her head around the encounter and how we generate revenue. I tried to explain it also by giving an example. “Say you do five fillings in one visit we are only going to get $100 (made up rate for educational purposes) for that encounter.” Am I explaining things correctly? I am thinking of switching to a clinic that primarily does fillings, exams, extractions, walk ins if time permits. There is a hygienist present and two dental assistants. I’m ok with not doing pros as I’m looking for a more relaxed environment at this point in my life. Curious what do some of you all schedule look like for those who work in similar clinics. How many procedures and exams are scheduled a day? They say on average would see about 10 patients. This is based on an 8hr day with an hour lunch.

This is more so for people who are not first timers applying for continuation contracts. I’ve been applying and getting them for the last few years. No issues. Things moved quickly. This year everything seems super slow. My existing contract ends in about 30 days and my application still says under review. I reached out and they told me it can take 30-90 days. Curious if anyone else is in the same boat. I submitted my application end of April. I wonder if them having limited staff is part of the problem. It’s never been this slow.

Our senior leadership wants to look into possibly partnering with another organization that sees a lot of autistic children. That organization currently has built a dental program for their autistic clients, but I believe it may not be going well. Mind you we don’t see a lot of children in our existing clinic. Most of our patient s are adults with behavioral health needs. I am open to learning more about the organization that sees a lot of autistic children. If your clinic sees a lot of children with developmental disorders can you provide a day to day for the provider? Is it a pediatric provider or a general dentist who has a lot experience with children? Our existing providers do not work with children often so management is fine with bringing on docs who do if we move forward with this. Are you all offering nitrous or any sort of in office sedation services?
