What’s next to try?

I’ve had primary provoked vestibuldynia my whole life. I’m really not sure what else to try?

- Multiple years of therapy
- 1 year of physio (which has 90% cured any underlying vaginismus - penetration itself is fine)
- I have hypermobility but no other underlying health conditions like PCOS
- No infections
- No skin conditions or allergic reactions
- Hormone tested and already tried oestrogen creams

The pain is all on the vestibule, mainly at 5 & 7 o’clock, but the rest is a bit sensitive. Only painful when provoked.

Lidocaine temporarily reduces the pain by 80% which feels promising, but it leaves a couple of spots that are sore, and I’ve read lidocaine isn’t a long term solution?

So what’s my next option and what might be the underlying cause? I recently went to a UK specialist but they couldn’t give me concrete answers.
