Bed mesh help

Got a Neptune 4 max I’ve been fighting with for few months. (See post history) Has been having issues with the extruder clogging after knocking prints off and carrying them around the bed. For history, I’ve had to replace 2 entire extruder heads, and the X belt, as my tensioner was faulty and frayed the hell out of my old one.

Reset everything to 0, let it warm up on standard PLA temp for 30 minutes then auxiliary leveled all 6 screws for 3 total passes on each to dial it in. Did the auto leveling (tramming) and this is the bed mesh. The past few times I’ve done this then adjusted each screw, I’ve thrown the bed mesh off completely and I’m never able to get it within the +/- .2 margin.

I showed a buddy and he said my bed might be warped, how do I go about testing to confirm bed warping (outside this) and fixing this? And the z offset is accurate, I did the paper test and adjusted for very little friction. Need some help before I go to customer service for the 4th time.

Edit: reddit doesn't seem to like me today and wont attach images. Ones of my bed mesh in fluidd, all 4 corners are above .5 and the center is at 0.0. Second is was the numbers for each point from leveling from the printers screen.
