What did I do wrong fusing up these MJG initiators?

During this year's show, I had 3 out of 17 MJG fuse tube initiators that didn't successfully light the fuses, even though the initiators went off. The pictures show the fuses the way I found them after the show. The initiators and the tape were physically separated from the fuses, and the fuses looked slightly burned, but hadn't fully ignited. The fuses were all newly purchased from Pyro Creations, and were definitely kept dry. I also included pictures of the way I connected the fuses to the initiators. I cut the fuses on a diagonal and used about 2" of 2" IPG paper tape wrapped around the fuse and initiator.

What am I doing wrong? Or what techniques do you guys use to connect MJGs to a fuse?

One thing I did notice was that the fuses fit a bit snuggly in the tubes - is there a chance they just blew out of the tube when the initiator fired before they could ignite? If so what should I do to stop that?

Designed and built my own racks this year

Basing the design around CNC cut plywood made the build very simple (especially helpful for the angled tubes), and 1x pine framing keeps the racks reasonably light. One of my goals was to make them stackable for easy storage. I've built 4 so far.
