AHI vs Flow Limitation

Hi guys - I am struggling with a conundrum that I am hoping you all can help me. I have tried 2 masks till now - Resmed P10 and Resmed F30i. I was hoping the f30i would have been a good fit for me since I am a mouth breather and i wanted to move away from the mouth tape + Chinstrap setup I had with p10.

My worry is that the machine is essentially maxing out when I am using f30i despite 0 leaks. the ahi is less than 5 but the flow limitation chart is also pretty hectic. Am I overthinking it? I am pasting 2 oscar screenshots of nights with similar AHI and hoping for recommendations around if i should move back to a p10 (5th June night) or continue with f30i (27th July night).

F30i vs P10 – Which would you stick with in my situation?

I have been going back and forth between the ResMed AirFit F30i and the P10, and I'm genuinely torn.

F30i

- Lower AHI (usually around 3–4).

- But as the night goes on, I develop increasing leaks from the sides of the mask. Good nights leaks are less than 5, but some nights have 20+ as well with the exact same setup and strap positions.

- I still need a chin strap because my jaw drops.

- My machine also has to work much harder, with median pressure often close to 19–20 cmH₂O.

P10

- Lower pressures (95% pressure usually around 15–16 cmH₂O).

- I need both a chin strap and mouth tape to prevent mouth leaks.

- AHI is a bit higher, usually 4.5–6.

I've also tried using a cervical collar, but that was by far my worst night—my AHI jumped to around 20–22, and leaks increased significantly, so that's not an option for me.

So my question is: Would you prioritize the lower AHI of the F30i despite the higher pressures and increasing leaks, or the lower pressures of the P10 despite the slightly higher AHI and the need for mouth tape?

For those who've used both masks or dealt with similar issues, which setup ultimately left you feeling better during the day? That's what I'm trying to optimize—not just the AHI.

Need help with setting and machine recommendations

38M, BMI 24.5, severe OSA diagnosed via PSG April 2026.

PSG baseline:

  • AHI 56.1, purely obstructive (zero centrals, zero mixed)
  • NREM-dominant — NREM AHI 59, REM AHI only 2.6
  • SpO2 nadir 80%, REM depleted to 5.1% of TST
  • No positional component

The journey

Tried fixed BiPAP S/T (EPAP 14/IPAP 17) — obstructive suppressed well but hypopneas were terrible (HI 6–12). PS of 3 wasn't enough.

Tried DreamStation AutoBiPAP — CAI spiked to 6.57 one night, treatment-emergent centrals. Doctor switched me to AutoCPAP citing lower central risk.

Tried DreamStation AutoCPAP — min 4 was useless, took an hour to reach therapeutic range. Got to AHI 6.21 on best night with min 10 but the algorithm was aggressive — machine would ramp to 16–18 on exercise nights and trigger centrals.

Switched to AirSense 10 AutoSet (Min 10, Max 18, EPR Full Time 3, Ramp Off) — this is where things clicked.

8-night average: 5.37. Clean nights average: ~4.1.

HI is essentially zero on all nights — EPR 3 solving hypopneas completely. Residual AHI is almost entirely obstructive apneas (AI 3–4/hr). CAI well controlled on non-illness, non-exercise nights (0.74–1.29).

Pressure line sits naturally at 11–13 most nights with a clean sawtooth pattern. Nothing like the sustained 16–18 blasting I saw on DreamStation.

1. Settings — anything left to try?

Is there anything I can try to improve the AHI further? I know it is below 5 but I still don't wake up fresh and sometimes I wake up with a migraine like headache.

2. Machine purchase — AirSense 10 vs AirCurve 10 VAuto

  • AirSense 10 AutoSet (with humidifier + heated tube): ₹52,000
  • AirSense 11 AutoSet: ₹67,000
  • AirCurve 10 VAuto: ₹67,000

AirSense 10 is proven for my profile. The VAuto runs a different algorithm — and my DreamStation AutoBiPAP experience was my worst period (CAI 6.57). AirSense 11 is same price as VAuto but same algorithm as the 10 I'm already on. I am thinking what makes sense and I am trying to think more long term since this is a significant purchase and will be my machine for the next 6-8 years at least.

Is there a compelling reason to go VAuto at ₹67k given my BiPAP history and that AutoSet is already working? Or does AirSense 10 at ₹52k make the most sense?

I recently underwent a sleep study and the results were highly surprising. My watch detected 8% deep sleep vs 32% as per sleep study. Similarly, everything else was highly innacurate.

While I do understand that these won't be 100% accurate and should ideally be used for directional understanding, what the sleep study showed me is how highly innacurate they are. The amount of waking up, movements, sleep stages.. Everything was innacurate. The only accurate thing was the total sleep time.
