Dizzy every day for weeks, heart rate involved but no one agrees it's the cause

44/f, otherwise healthy (lost 60 lbs this past year, work out regularly). About 3 weeks ago I started having daily dizziness - sometimes described best as "swimmy,". Along with it: fatigue, brain fog, shortness of breath, sometimes blurred vision.

Quick background: my resting heart rate has been in the mid-40s for months with zero symptoms, so when this started it seemed unrelated at first. Then I had an ER visit because it felt like I was about to pass out. EKG showed a heart rate of 46, and I was diagnosed with "symptomatic bradycardia." But an electrophysiologist I saw afterward wasn't convinced the heart rate is actually low enough to explain what I'm feeling.

Since then I've been trying to figure out what else it could be. A few things:

  • It's daily, not random - happens whether I'm resting or up and about
  • No fainting (other than the one ER episode), no chest pain otherwise, no palpitations
  • All tests, imaging, and bloodwork I've done have come back clear
  • Severity seems to come and go

Not looking for a diagnosis, just want to hear if this pattern is familiar to anyone, and what eventually helped or explained it for you.

Resting HR in mid-40s, EP not convinced it's causing my symptoms - anyone been through this?

44F, otherwise healthy (lost 60lbs over the past year, work out regularly, eat well). Resting heart rate has been in the mid-40s for about 7 months. Been feeling just fatigue until recently.

About 3 weeks ago I started having daily dizziness, worse fatigue, brain fog, blurred vision and shortness of breath. Ended up in the ER after I almost passed out - EKG showed resting heart rate of 45 BPM, troponin negative, blood work and imaging all clear. Diagnosed with "symptomatic bradycardia," discharged with a 14-day Holter and an EP referral.

Saw the Electrophysiologist, who doesn't think my heart rate is low enough to be causing what I'm feeling. I'm now looking into other possible causes for my symptoms but wanted to ask here too:

  • Has anyone had a similar resting heart rate (mid-40s, otherwise asymptomatic-looking on paper) end up being the actual cause of dizziness/fatigue/shortness of breath, and gotten a pacemaker?
  • If so, what did it take to get taken seriously - a loop recorder catching an event, a specific test, a second opinion?
  • For those where it wasn't the heart rate - what else did it end up being?

Not looking for a diagnosis, just curious whether "the number alone isn't low enough" has been the final word for others in a similar spot, or whether it usually takes more digging.
