Off-cycle resident graduating Feb 2027 — Conrad 30 timeline?

I’m an off-cycle resident and will graduate around mid-February 2027. I finally found a job that is willing to sponsor me for a Conrad 30 waiver, and they plan to submit the application on October 1, 2026.

For anyone who has gone through this process, is that enough time to get the Conrad 30 waiver, DOS recommendation, USCIS approval, and H-1B processed before I need to leave the U.S.?

I’m not sure whether my DS-2019 can be extended through the August 2027 board exam. If it cannot be extended, my current J-1 would end around mid-February, with the 30-day grace period ending around mid-March 2027.

Has anyone had a similar timeline, especially as an off-cycle resident? How long did the entire Conrad 30 → DOS → USCIS → H-1B process take for you?

J1 waiver job? - March 2027

I’m looking for a J-1 waiver day hospitalist position starting in March 2027. I’ve been emailing recruiters and applying online, but I haven’t had much luck so far. Most of the opportunities I’m finding are for nocturnist positions, primary care or open icu.

My preference is a day hospitalist role with a closed ICU. I’m open to different states and waiver programs if it’s a good fit.

If anyone knows of hospitals that are hiring, or has any recommendations, I’d really appreciate it. Thanks!
