Locked knee question

Hey, I've had my first ACL/meniscus surgery when I was 17, now I'm 37.
I did a my rehab and 1 year later I retore my ACL. On the advice of the surgeon, I did not get another surgery. I've been without an ACL on my right knee for 20 years without any major issues. I did powerlifting, I'm currently running ultras. From time to time I get this "locked knee" situation where I have to push down on my knee cap and extend my leg to put everything back in place. As it can be seen on the attached photo something is bulging and displacing the knee cap.
The photo is 1 week old. I have an MRI from 1 month ago and it doesn't show any meniscus damage. I had 3 different orthopedic surgeons look at it and they all said the same - no cartilage damage. Like I've said, I've been with a torn ACL for 20 years, during which that time this happened usually several times per year. As of the start of 2026, it's happening almost every week. It's usually happening when I'm walking and the muscles are relaxed. It's funny how I ran a 75km ultra with 4500D+ and didn't happen once.
Anyone experiencing anything similar? I would like to avoid another surgery, as when this happens I can fix it in 5 seconds.
