Does anyone have this one down

Now, just to be clear, I understand that the way out of this is to move away right away and I've also completed this Color Quest entirely. So I'm all good on that front. But it's weird that not only was I spawned directly next to the obstacle, but that when I died, I wasn't given the option to revive. The game just went on without the character alive. This wasn't a one time glitch either. It happened multiple times and I actually have another video of it. I just wanted to see if this happened to anyone else out of curiosity.

Anybody know how to solve this? Been trying for so long now :/
