Test pain

I’ve been using TRT Clinic prescribed Test C 200mg/ml for about 6 months with a little injection site irritation lasting maybe 24 hours.

I recently changed over to 300mg/ml Test E from a different company and noticed severe injection site swelling and pain that last a week. Is the Test E bunk or is this normal when switching to a more potent test?

1 Ml per week for both split Biweekly. Buttock, left thigh,right thigh.

My total testosterone levels are at 967! I haven't felt this great in years and that's including being in testosterone for years. I've starting injecting three days a week instead of one big dose once. We did increase my dose from .8ml to 1.2ml a week. My strength is still down but energy and sleep have improved. I'm still heavier than i want to be but working on losing wieght. Blood pressure is in the 120s/70s, blood sugar is in normal range, blood count is in normal range. Liver enzymes are elevated a little so we are watching that and the doctor is gong to call back in a discussion with that. Finding a doctor that actually helps us a game changer.

I´m a 34y old guy, 200.6 lb, 6’1” tall

I´ve been on TRT for 13 years (just testosterone enanthate, nothing more), for the most part i´ve been using around 200mg weekly, sometimes more than that but for the last 2-3 years only on that dosage weekly.

Over the past months (or even 1-2 years, I’m not sure), I’ve been experiencing symptoms of constant anxiety (as if my mind and body are 24/7 alert to potential threats), feeling my heart beating strongly all the time even though my blood pressure and heart rate are normal, severe insomnia (waking up 5–10 times during the night), brain fog upon waking that lasts throughout the day, headaches when I try to work, inability to function cognitively at a normal level, chronic fatigue, etc. It has reached a point where I need to find a solution. I’m not even able to have a coherent conversation with someone because I feel constantly on edge. I could go on, but I think this is enough to give you an idea.

It feels like my body is in a state of constant hyperactivation 24/7. About a week ago, I started thinking it might be related to testosterone, so I did a blood test. These are the results.

I inject 200 mg of testosterone enanthate every Monday (once per week). The blood test was taken on day 7, just before my next injection (i.e., 7 days after the last injection):

FREE Testosterone: 102.11 pg/mL

TOTAL Testosterone: 1106 ng/dL <- So it will be significantly higher in the days following the injection?

SHBG: 14.9 nmol/L <- Low SHBG means a lot of free testosterona that leads to high estrogen if I understand currently

Estradiol (E2): 67 pg/mL <- So it will be significantly higher in the days following the injection?

Hematrocrit: 50,7% <- high

hemoglobine: 17,4 g/dl <- in range

So my first thought is that if my estradiol is elevated at trough, it’s likely even higher during the first days after the injection (peak), which could be part of the cause of my symptoms.

Also my hematrocrit is in the higher end, that could be because of testosterone and maybe because my blood is ticker then I get brain fog and headaches

Also during the night I wake up many times and with headaches, it could be sleep apnea too (but I do not snore)

I also measured:

Progesterone: <0.50 ng/mL

(Based on the lab reference ranges, this is above the male lower limit of <0.2 ng/mL, so it may still be within range)

DHEA-S: 308.40 µg/dL

My idea is to add pregnenolone supplementation, since although it appears within range in the lab results, being on testosterone replacement therapy it might be functionally low for me (especially considering that my testicular production is essentially suppressed).

I am also considering reducing testosterone to 125 mg per week and splitting it into two injections, as well as adding 250 IU of HCG twice per week for neurosteroid support and to prevent testicular atrophy (“shrinkage”).

I have a few questions:

If estradiol is elevated at trough, does that mean it is likely even higher in the first days after the injection? Could this be part of the cause of my symptoms?

Would you recommend reducing the dose, splitting injections, and adding pregnenolone supplementation given my SHGB is low and I´m prone to aromatice lots of E2 that may be causing the symptoms?
