Tell me what happened to my driveway. Ready, set, GO!!!!!!

Our driveway was poured on February 28th of last year, and it’s already starting to disintegrate. The photos are from a few weeks ago, but honestly it seems to be getting worse by the day and is now spreading to other areas of the driveway.
There are literal depressions forming where the concrete has broken down, and rocks — some as big as marbles — are coming loose from the surface. It almost looks like parts of the concrete are just crumbling away.
We had an extremely mild winter with very little snow, and we did NOT use any ice melt or salt on it this past winter.
A few questions for anyone with concrete experience:
What could cause this in such a new driveway?
Is this normal at all for a driveway barely a year old?
Is there any way to fix or stop it from getting worse?
How long should a newly poured driveway realistically last?
The contractor originally told us the driveway should last at least 10–12 years when he was trying to get us to agree to the job. Now that we’re having issues, he says there’s no warranty and he doesn’t really have any responsibility to fix it.
Just trying to understand whether this looks like bad concrete, improper mixing/pouring/finishing, weather damage, or something else entirely. Any insight would be appreciated.

Bruker XRD

Who is using the Bruker? Do you have any advice?

I’m having problems with this mineral. It’s not in the database, and I have no idea how to add it…
