Medical Records Nightmare

The records were disclosed by the hospital in one 2.271 page PDF file. I meticulously split into 149 files per section data.

I’ve lost three-weeks with zero productivity due to AI. Four different platforms were used as well as their top-tier subscriptions. I’ll refrain from complaining about AI & instead ask for guidance or advice.

My biggest issues are accurately extracting data, merging that data with other data, and then compiling all of it into date by date records.

The patient was hospitalized for approximately three-weeks. Of those days, thirteen are of relevance regarding poor communications and questionable orders. Data derived from those days will serve as validation to my statements as well as reason for questions that I intend to pose.

I’ve already passed through the patient relations and compliance departments. Their responses were identical, “Per our internal investigation and review of the medical records, we’re confident that the patient received the normal standard of care”.

The problem with their claims are that records were reviewed by a cardiologist directly involved with the patient’s care in conjunction with the records containing errors and omissions.

Should anyone have any suggestions or recommendations, I’d be extremely grateful.
