Mystery itchy bumps spreading for 7+ days, dermatologist unsure, please help

I’ve had the symptoms shown in the first two pictures (on my right leg only) for about 7 days. I initially assumed it was from sand, sweat, and sunscreen since we got back from a vacation in Italy on August 6th. About 9 days in, my boyfriend developed similar-looking bumps on his stomach.

On Monday I saw a dermatologist and she said it could be shingles (herpes zoster) or contact dermatitis, and prescribed Advantan cream and Aerius (desloratadine). It looked a bit better for a day, but since Tuesday I’ve gotten many, many more bumps all over my hands and legs (both, right and left). These new ones feel different: itchy AND painful.

I went back today and she added Xyzal (levocetirizine) on top of the Aerius. She said the bumps look like insect bites, so I checked the whole house for bed bugs and found nothing. Until tonight I hadn’t noticed much itching in the evenings, but tonight it suddenly got way worse.

What should I do at this point?

Looking for advice - persistent scabies despite multiple treatments

First ever Reddit post. I’m at my wits’ end with what appears to be persistent scabies and would really appreciate advice from anyone who has been through something similar.

I’ve had multiple rounds of permethrin, ivermectin and malathion, with no lasting success. I’ve seen my GP around 8 times, been referred to dermatology, seen a private GP, contacted 111, and basically spoken to any healthcare professional who would listen. I keep getting given the same treatments, but the problem keeps coming back.
The pattern is usually: treatment, things improve for a couple of days, 2–4 days later the itching starts again, bumps develop, then I start seeing what appear to be new burrows.

I’ve also been extremely thorough with environmental measures like washing clothes, bedding and towels at 90C, vacuuming daily and dealing with anything I can’t wash. I’ve treated repeatedly, so I genuinely don’t know what else I can do.

This has also started to really affect my mental health. The constant itching, lack of sleep, uncertainty and feeling like I’m never getting rid of it is becoming incredibly difficult to cope with. I’m exhausted and honestly desperate for an answer rather than another identical course of treatment.
Has anyone experienced genuinely treatment-resistant/recurrent scabies after permethrin, ivermectin AND malathion, particularly where symptoms drastically ease then seem to restart within a few days of treatment?

What eventually worked for you? I’m not looking for a diagnosis from Reddit I’m just hoping someone who’s been through this can point me towards something I haven’t already tried or an avenue I haven’t thought of. Thank you for taking the time to read all of that I appreciate it was very long, cant sleep with the itching and typing this out seemed to help distract me.

NP said scabies ER doctor said it’s not

Woke up in the morning to crazy itching. Everytime I would itch welts would pop up. The walk in NP said it was most likely scabies because the job I worked(Sheriff deputy) I didn’t believe it was even after applying the cream the prescribed me and steroid pack. I went to the ER and they said I was not scabies. What do you all think?

After 3 days of my last(2nd) dosage(ivermectin) some new nodules have started appearing, can't seem to find any burrows, they aren't itching(might be due to anti-histamine) is it possible reinfection

Cleaning and treatment

My fiance works at an apartment community and routinely deals with nasty situations once people move out or are evicted. About 2 months ago he started having issues on his legs including being very itchy and scratching his skin until it created sores. He suffered, but he didn't really say too much about it other than it itched him mostly at night. I think he was trying to minimize what he was going through. Unfortunately he suffered like that for over a month until the sores eventually became impetigo. We went to Urgent Care and they gave him some creams and told him it was impetigo. Obviously those creams didn't work and it continued to get a little bit worse. About 12 days ago he was diagnosed with scabies, which is both of our worst nightmares. They prescribed Permethrin cream, and said it was enough for both of us to do a full treatment. There was definitely not enough cream for both of us to do a treatment, so I got on the Telehealth and got my own prescription for the cream. We also both did Telehealth and got Ivermectin pills, and they gave us enough to do two rounds. The cream failed and we did take the Ivermectin last Friday. It advised that we needed to do a second round of ivermectin in 2 weeks. Everything that I see on here says to do the second dose after 7 to 10 days. Also I want to mention that I stayed up all night cleaning just about everything I could think of. I used Super Soap which has spinosad in it. I also got a steamer and attempted to use that. I guess I wasn't thorough enough because it has slowly started itching and causing more issues again. I especially feel bad for my fiance because he has to go to work and go outside for an hour or two a day where it is almost unbearable for him. I have major medical issues including a weakened immune system, and he has very thin and sensitive skin on his legs because he was burned when he was young. We have gone all in because we plan to retreat again on Saturday. We have ordered spinosad and benzyl benzoate from the maximumpulse website. We've also gotten the full kit of mite be gone, including laundry additive, pet spray and home spray. I've also got nu stock sulfur cream and we've made up our own sulfur lotion. I also plan to mix together tea tree oil, coconut oil and a little clove oil to have one hand to soothe after the treatment. I'm really looking to see if anyone can tell me if there's anything that we're not thinking of or something that may be effective that we haven't purchased. I am very stressed about the cleaning portion because although I feel like I did a pretty thorough job last time, obviously it wasn't enough. What did you use to clean your environment and your home? Were there things that you initially didn't think about, that maybe important to make sure I address? I didn't include things like replacing our deodorants or toothbrushes. This time I fully intend to replace things like that. I'm also not sure if I did everything in the proper order. We took the Ivermectin around 6:00 p.m., and I stayed up all that night cleaning. When my fiance woke up the next morning I had everything ready including clean bedding, towels and things like that. I didn't strip the bed the night before. I think that was a mistake because even though the Ivermectin was still working in my system the next morning, I was so itchy after pulling everything off the bed. We do have a very thick mattress along with a memory foam mattress topper and mattress protector. What steps did you take as far as cleaning? Did you change the bed out right before you did your treatment, and then change it out again the next morning or how did you handle that? Please , tell me exactly what you did and what order you did it . I want to ensure that we take care of this because my fiance is having an especially hard time. Since it's so hot here in florida, he's forced to wear shorts and you can clearly see how bad his legs are. He's embarrassed to even be at work right now. It makes me so angry to think that the people didn't even care who might have had scabies and passed it on to us. We are pretty clean people, and this is really our worst nightmare. Please help me and give me all the recommendations for steps of cleaning as well as what you used. Is it even worth it to keep the steamer that I have? We do have dogs and I know that poses an issue as well. I do plan to spray them and their bedding down with the mite be gone pet spray. I'm so sorry to anyone that's gone through this, and especially the people that have gone through it for long periods of time. I can't even imagine the mental toll that must take.

Just answer doctor said i have scabbies but not sure to trust or not

First i have intense itching from sweating and working in a care home since 2 weeks ago and it is literally heatwave in the uk , not sure if it is the polyester trousers making me having a rash.

Dont have intense itch at night , just as the same as normal. Then these tear appear after i scratch them.

And i dont have any pimple like bumps or the burrow lines

Please help and give advice

New stomach skin rash.. help!?

50 yrs old, Female, Immunocompromised, Been battling 7 months in bed, 3 dermatologists that broke me down. Started with dry eyes, demodex blepharitis, full blown demodicosis, to now hair loss, things biting me bad, go through different rash/mite phases. These pictures are most concerning. They are new on my stomach ( entire body's broken out) but these bumps are new. Im in a flare up now so my big bag dance parry hairs are going off, my ears are burning and bright red, my eyes and mouth so dry. I checked for bed bugs. Could this be something else... ive researched a lot about scabies but dont think k so??? Morgallons 1000% and on fire because of the flare up kicking in. My scalp has malezzia and ive tried everything. Everything ive read about shingles well the newer stomach rash doesn't hurt. My entire body is on fire in pain, head itching like crazy, fibers are breaking out, my face is absolutely wrecked with inflammation and mass break out.

Coming out of my eyes, ears, bellybutton, completely covered head to toe inside and out. Don't sleep.average 3 nights a week. I will NOT go back to a dermatologist- unbelievable, gaslighting, demeaning, never even used a derma scope, all but psychiatric... They're awful, just to save face.... while laid up in bed 7 months in horror.

Please will anyone weigh in on thus new stomach rash? Thank you!

Tea tree oil

I am on my first dose of ivermectin and will take another tablet in 7 days, I heard that tea tree oil kills mites, any recommendations on how to use tea tree oil while I do my treatment ?

Is this scabies

At first I thought they were mosquitos bites as this is how they usually look on me but then more and more kept appearing. I usually only itch bad twice through out the day and earlier today I seen some little critters crawling on my shirt today. I put on a cream to help reduce inflammation and itchiness because of how bad it was earlier. Today is the first time they appear on my neck

so i got reinfested and doing 2 rounds of ivermectin+the cream again with all the washing and cleaning etc. but i feel that maybe they dont all die, is it bad to do 3 rounds? all with 7 days in between?

I am positive (most likely) that I’m showing early symptoms of Scabies but my doctor will not help

I (F21) live in Canada and I started noticing my wrists itch with white lines on them. Photo attached. I thought it was just my Psoriasis flaring up.

Before coming back to Dubai to visit family, I started itching all over the body. I immediately took out psoriasis as the cause because I have never iched all over because of it.

Naturally, I ask my mom if we could book an appointment and speak to a dermatologist regarding what it could be and she prescribed me reactine.

2 weeks later and I was still itching all over my body but worse and I should have told her maybe its not an allergy because the reactine is not helping but again, she dismissed it as an “allergic reaction” and upped the dose of the reactine.

I really dont know what to do because now my little sister is itching too and from what Im reading on this reddit its not long till it gets worse for the whole family :(

I don’t know if I should book an appointment and insist on another look or if I can use something over the counter here in Dubai?

Please help!

Side note: Im Inclined to think this isn’t an allergic reaction because reactine isn’t helping at all and Im itching all night, unable to sleep now.

Has anyone used spinosad? Does it really kill scabies eggs? Were you able to cure yourself with it alone? How can it be supplied to other countries?

Scabies came back...

After dealing with scabies for over a year I finally managed to get rid of it. It was pure hell and took my sanity. Unfortunately now about 4 months later, just as I was getting used to getting my life back and going outside again, I got it again.

It came back fierce and quickly spread to multiple places.

I can not deal with this again in this horrible mental state I'm in and thinking about ending it all today. I gotta go to work in an hour with my arms and hands looking like a scabies mess.. I don't think I can deal with this..

Urgent Call for the Development of a Scabies Vaccine

Dear researchers, parasitologists, dermatologists, and vaccine-development specialists,
We are calling on the scientific community to make the development of a vaccine against scabies a research priority.
Scabies is caused by the parasitic mite Sarcoptes scabiei and remains a significant global health problem. Scientific literature has already highlighted the need for vaccine development, while reduced susceptibility to some existing acaricidal treatments is an increasing concern. At present, however, there is no licensed human vaccine against scabies.
What we propose
We propose launching an accelerated international research program, ScabiVax-NG, focused on developing a multicomponent vaccine that targets several stages of the mite’s life cycle:
egg → larva → nymph → adult mite → reproduction.
Particular attention should be given to antigens associated with:
egg development and hatching;
mite feeding;
immune evasion;
larval and nymphal development;
reproduction of adult mites.
There is already preliminary experimental evidence supporting this direction. In a rabbit study, a combination of three recombinant S. scabiei proteins — serpin, CLP5, and CLP12 — produced significant protection and substantially reduced mite numbers.
Proposed six-month research phase
We are not asking researchers to bypass the necessary safety requirements. We are asking for the research phase to be accelerated as much as scientifically and ethically possible.
Month 1: Analyze the S. scabiei genome and proteome and identify a panel of promising vaccine antigens.
Months 2–3: Experimentally evaluate the most promising antigens and determine their ability to induce a specific immune response.
Months 3–4: Develop several multicomponent vaccine candidates.
Months 4–6: Conduct preclinical evaluation of the leading candidates, including their ability to reduce mite survival, reproduction, and completion of the life cycle.
The main objective
Within six months, the goal should not be to promise a finished vaccine for public use. The goal should be to identify a scientifically validated lead vaccine candidate that can immediately proceed to the next stages of preclinical and clinical development.
Vaccine development should complement existing treatments, not replace them.
We urge the scientific community, universities, government institutions, and pharmaceutical companies to combine resources and expertise to address this problem.
Scabies is a parasitic disease for which humanity still has no vaccine. It is time to make vaccine development a priority.
Proposed goal:
ScabiVax-NG — a next-generation vaccine against Sarcoptes scabiei, designed to disrupt the mite’s life cycle and prevent reinfestation.
We need not promises, but an urgent scientific program, funding, and international collaboration.

Eczema and or active/post scabies?

Applied my first permethrin cream on the 2nd of august and went to get checked up and was told to apply a second permethrin cream on. In which I did on the 6th of august then applied a third one on the 13th of august. And these fluid bumps have started appearing 2-3 days ago and have been multiplying, transforming from red itchy bumps to these fluid bumps. I have another appointment booked to get checked up again two days from now. Pretty paranoid and worried about what this could be. If you have any recommendation of what to use in order to treat/relieve the itchiness please let me know!

possibility of reinfection

Hi, i had scabies back in november and was fully cured by january, it is now august. I went through my wardrobe the other day and tried on some clothes to sell for vinted, and ever since then my skin has had a strange rash. It’s not very itchy, and it is contained to my right arm, however I can’t help
to notice the rash resembles scabies. The other possible explanation is a bacterial infection,
since i’ve just started boxing and haven’t washed my wrist wraps yet.

18M, 60kg, 5'7 it's quite some time since I get these, they come for few hours and then go

So it comes when I feel hot then I feel itchy like very itchy everywhere and these comes wherever I feel itchy, it's common on my private area and now it's coming on many areas like hands back belly and mostly fingers, in fingers they get swollen not any bump
