Under-eye hollows: why does every clinic recommend something different?

I help international clients book clinic appointments in Korea, and under-eye hollows come up quite often.
What confuses people is that one clinic recommends filler, another says Juvelook, and another suggests Rejuran or Re2O Fine. They can all be used around the eyes, but they’re not really doing the same job.
Also, not every dark circle is caused by a hollow. Thin skin, pigmentation, visible blood vessels and eye bags can all create a similar tired-looking shadow.

This is how I now understand the main options:

Fat grafting (autologous fat transfer)
This may be worth considering when there is a real loss of volume extending from the under-eye area into the upper cheek. It uses your own fat and can last much longer than an injectable treatment.
The downside is more swelling and downtime. Some of the transferred fat may be absorbed, and the result can be uneven if the fat doesn’t settle consistently.

Hyaluronic acid tear trough filler
Filler gives the quickest change and works well for some mild to moderate hollows. It can also be dissolved if the result isn’t right.
The under-eye area is quite unforgiving, though. Swelling, lumps, a bluish tint or migration can happen, especially if too much is used. Vascular complications are rare but serious.

Restylane Eyelight
This is an HA filler, but it isn’t simply a regular filler being used under the eyes. It was specifically developed and studied for under-eye hollows.
It uses NASHA technology, creating a supportive gel that absorbs relatively little water. That can be useful in an area where some fillers may attract water and leave the eyes looking puffy. The result is immediate and may last up to 18 months for some patients.
It still has the usual risks of under-eye filler and won’t fix pigmentation or prominent eye bags.

Elravie Re2O Fine
This is a newer ECM-based injectable designed for delicate areas with thin skin. It seems more focused on tissue quality and gradual skin improvement than simply adding volume.
It may soften a shallow hollow, but I wouldn’t expect it to fill a deep tear trough in the same way as filler or fat grafting. As it is still quite new, there is also less long-term clinical information available.

Juvelook (often called Juvelook Eye at clinics)
Juvelook combines PDLLA and hyaluronic acid. The change is gradual because it works partly by encouraging collagen production.
It may help with fine lines, elasticity and mild hollowing, but it usually takes several sessions and won’t give the same immediate volume as filler. Temporary swelling and bruising can happen, and proper injection technique matters.

REJURAN i
Rejuran i is a polynucleotide treatment made for the delicate skin around the eyes. It is more suitable for thin, crepey skin, fine lines and texture than for replacing lost volume.
The skin may gradually look healthier, but a deep hollow will still be there. It can also be painful, and small bumps or swelling are common for a few days after treatment.
The rough way I separate them is:
Real volume loss: filler or fat grafting
Thin skin and fine lines: Re2O Fine, Juvelook or Rejuran i
Eye bags creating a shadow: ask about fat repositioning or lower eyelid surgery
Pigmented or vascular dark circles: none of these may fully correct the problem

There isn’t one treatment that is automatically best for everyone. The right option depends on what is actually creating the hollow or shadow in the first place.
