Stuck here need help im applying for toc in physics and chemistry and will give exam in maths English and data entry operations so how do i fill this please help

So I have failed my class 12th exam in maths and I want to give on demand exam for maths and I am filling the form here so it is asking me your previous qualifications so what should I fill here class 10th or 12th it is also saying that the option I fill in the section of your previous qualification I have to upload the marksheet of it so what to do please help
