Low-latency 3D H SBS HDMI capture → Apple Vision Pro setup for surgery use?

Hi everyone,

I’m looking for real-world setups to build a very low-latency video pipeline for surgical use.

The idea is to take a standard HDMI side-by-side (SBS) 3D output from a surgical imaging system and display it on Apple Vision Pro.

Main constraint is critical for the OR context: 👉 minimum possible latency, ideally ≤ 20 ms max

What I’m trying to understand:

  • Best way to capture an HDMI SBS signal for near real-time use (capture card vs other methods)
  • Best pipeline to send and display it on Apple Vision Pro
  • Whether a MacBook Pro + capture card setup can realistically achieve ultra-low latency
  • Any macOS / visionOS apps that can reliably handle this type of feed
  • Whether anyone has tested this in a real surgical environment with stable performance
  • Practical limitations in terms of latency, compression, or frame buffering

Goal: HDMI SBS input → Apple Vision Pro with the lowest possible delay and stable frame rate, suitable for surgical viewing/teaching

Any feedback from people who have actually tried similar setups would be really appreciated.
